Obtain medical treatment immediately, and tell your
doctor that your injury is work related. (No claim can
be established without a medical record stating that
the injury was work related.)

Report the accident to your supervisor/employer as soon
as possible.

Note the exact time and location of your accident.

Obtain the names and addresses of all witnesses, if
any. (Do not be concerned if there are no witnesses.
Having a witness is not essential to your right to file
or establish a claim.)
5.
Don’t speak to anyone else about your claim, except
your doctor.
